This website, sample.fmenergycenter.com, is for those who have purchased access to Sample Wellness Practice’s Functional Medicine Program and entered into a Functional Medicine Program Agreement with Sample Wellness Practice. A subscription is required. Unauthorized access is prohibited.

Payment for access to this site is paid in advance monthly, starting on the date access is purchased. After access is purchased, no refunds will be provided. Access renews monthly, unless access is cancelled in advance of renewal. Once access is renewed, no refunds will be provided, even if users do not access this site.

Sample Wellness Practice reserves the right to refuse access at their sole discretion. Access is for one log-in. Sharing access may be grounds for immediate termination of access and the Functional Medicine Program Agreement. Unless otherwise attributed, all content is the property and work product of Sample Wellness Practice and is intended solely for the use of Clients. Clients are not allowed to distribute content. Sample Wellness Practice and other contributors reserve all rights to pursue damages from violation of this policy.

Clients purchase content on an “as is” basis. Additional content may be provided at the sole discretion of Sample Wellness Practice and may be added, altered or removed at any time. Content access may be discontinued at any time, with no further payments accepted. Sample Wellness Practice may change the price of content access to be effective on sign up or renewal, with notice given to renewing clients at least thirty (30) days in advance or in accordance with the Sample Wellness Practice’s Functional Medicine Program Agreement.

The laws of the state of Nevada shall govern an disputes or issues related to this site, the construction of its terms and the interpretation of the rights and duties of the parties hereto.
